Fewer no-shows: a WhatsApp reminder and reschedule playbook for clinics

An empty chair at 11:30 is lost time you can't sell again. A reminder the patient can answer with one tap fills more of those chairs than a receptionist ringing through a list at 6 pm.

The short version

  • A Cochrane review of eight trials found text-message reminders raised attendance compared with no reminder, and worked about as well as phone-call reminders. The evidence is SMS, not WhatsApp, and rated low to moderate quality.
  • Send two messages at most: a confirmation when the appointment is booked and a reminder the day before, each with Confirm and Reschedule buttons.
  • Appointment reminders are utility templates at ₹0.115 per delivered message in India, before GST. Add an offer or a health tip and Meta can price the whole template as marketing.
  • Keep diagnosis, test names and treatment out of every template. Lock screens and shared family phones show more than you think.

Gurpreet Kaur manages the front desk of a three-chair dental clinic in Ludhiana. Every evening at six she opens the appointment diary and starts calling tomorrow’s patients. About half pick up. Of the rest, some call back during the first appointment of the morning, and some simply don’t come. By the time the chair is empty at 11:30, it’s too late to offer it to anyone.

A WhatsApp reminder won’t make every patient turn up. It does let a patient who can’t make it say so with one tap the night before, which gives the clinic time to fill the slot.

What the evidence says

The best summary we know of is a Cochrane review from 2013 that pooled eight randomised trials with 6,615 participants. It found that text-message reminders increased attendance at healthcare appointments compared with no reminder, and worked about as well as phone-call reminders. The authors rated the evidence low to moderate quality, and the trials used SMS, not WhatsApp.

So we won’t promise a number. Measure your own no-show rate for four weeks before you start and four weeks after. That’s the figure that matters for your clinic.

Meta requires opt-in before a business messages someone on WhatsApp. The opt-in must make clear the person is agreeing to messages and must name your business. It can be collected any way you like, including on paper.

For a clinic, the natural place is the registration form or the booking call: “We’ll send your appointment reminders on WhatsApp from Smile Studio Dental. Is this the right number?” Write down the date and who took it.

Ask whose phone it is. A child’s appointment goes to a parent. An elderly patient’s reminders often go to a son or daughter in another city. Record the number you’ll actually message and the patient it’s for, so the reminder names the right person.

Step 2: send two messages, not five

Our suggested timing for most outpatient clinics:

WhenMessageWhy
When the appointment is bookedConfirmation with date, time, doctor and addressCatches wrong dates while the patient is still on the line
The evening beforeReminder with Confirm and Reschedule buttonsLate enough to be remembered, early enough to refill the slot

That’s our view, not a Meta rule. A clinic with procedures that need fasting or preparation may add a message two days before with those instructions. A walk-in-heavy clinic may skip the booking confirmation. What we’d avoid is a third and fourth reminder: each one costs money, and patients start muting you.

The evening reminder also counts against your messaging limit, because it goes to people who haven’t written to you in the last 24 hours. New WhatsApp Business Platform accounts can reach 250 different people a day. A clinic seeing 40 patients a day is well inside that; a diagnostic chain sending 400 reminders a night needs the higher limit first. Our messaging limits guide covers how to get to 2,000.

Step 3: write a template Meta will price as utility

A reminder sent the evening before is outside the 24-hour window, so it must be an approved template. Meta’s category rules decide the price. A utility template must be non-promotional and either specific to the user or essential to them. An appointment reminder fits. In India a utility template costs ₹0.115 per delivered message, before GST, on the rate card effective 1 October 2026.

Here’s wording that stays inside those rules:

Hello {{1}}, this is a reminder of your appointment at {{2}} with {{3}} on {{4}} at {{5}}.

Please tap a button to confirm, or to choose another time.

Buttons: Confirm, Reschedule.

Where {{1}} is the patient’s first name, {{2}} the clinic, {{3}} the doctor, {{4}} the date and {{5}} the time. Meta’s template review rejects templates that start or end with a variable, which is why the greeting and the closing line are fixed text.

Mixed content becomes marketing. Meta’s rules say a template that combines a utility message with promotional content is categorised as marketing, and Meta can change a template’s category later if the messages don’t match it. So don’t add “20% off teeth whitening this month” or a monsoon health tip to the reminder. In India that moves it from ₹0.115 to ₹0.8631 per message and turns a service into an advert. Send offers, if you send them at all, as separate marketing templates to patients who agreed to hear about them.

Step 4: make rescheduling one tap

When a patient taps Reschedule, they’ve written to you, so the 24-hour window is open and you can reply without a template. Offer two or three real slots as buttons or a list. Don’t send “please call the clinic”; the patient has just told you they’d rather not.

  6. Smile Studio Dental:
Done. See you on Saturday at 11:00 am. The Thursday slot has been released. 9:05 am
The patient never had to call. The Thursday slot was free before the clinic opened.

The replies after the tap are service messages. The first 1,000 each month per number are free; after that they cost ₹0.115 each in India, before GST. Keep the exchange short.

If your booking software can’t take the new slot automatically, route every Reschedule tap to the front desk’s queue in a shared inbox, and have one person clear it first thing in the morning. Otherwise the patient arrives on Saturday to a diary that still has them down for Thursday.

Step 5: let patients call the WhatsApp number

Some patients, especially older ones, will always want to talk. If your number is on the WhatsApp Business Platform with calling switched on, they can tap the call icon in the chat and ring you on WhatsApp, and calls from customers are free to the business. Meta requires a messaging limit of at least 2,000 and a payment method on your WhatsApp account before calling can be enabled. Our WhatsApp Calling guide has the details, including the permission rules if the clinic wants to call a patient back.

What never goes in a reminder

A WhatsApp message shows up on a lock screen, in a notification on a smartwatch, and on a phone that three family members share. Write every template as if someone other than the patient will read it.

Leave out:

  • the diagnosis, suspected condition or reason for the visit;
  • test names, results or report links that open without a login;
  • treatment or procedure names (“root canal”, “IVF consultation”, “HIV test”);
  • the department, when the department name gives away the condition.

“Your appointment with Dr. Mehta on Thursday at 11:30” is enough. The patient knows why they’re coming.

The same care applies inside the chat. If a patient asks about a report, confirm who you’re talking to before discussing it, and don’t paste results into a thread that sits on a shared phone.

India’s Digital Personal Data Protection Rules were notified in November 2025, and most of their duties for businesses, including the detailed notice to people whose data you hold, apply from around May 2027. Treat that as time to prepare: record who consented and how, and decide how a patient would ask you to delete their details. This isn’t legal advice.

What it costs

A worked example: a clinic with 40 appointments a day, 26 working days a month, sends a booking confirmation and an evening reminder to each. That’s 2,080 utility templates a month: 2,080 × ₹0.115 = about ₹239, before GST. Replies to reschedule taps come out of the 1,000 free service messages per number first.

Compare that with the fee for one appointment that doesn’t happen.

Before you switch it on

  • Opt-in wording on the registration form, naming the clinic
  • The right number recorded for children and elderly patients
  • Two utility templates approved: booking confirmation and evening reminder
  • No diagnosis, test, treatment or department names in any template
  • Reschedule taps go to one named person’s queue
  • Calling hours set, if patients can call the WhatsApp number
  • Four weeks of no-show numbers written down, so you can compare

Common questions

Do WhatsApp appointment reminders reduce no-shows?

A Cochrane review from 2013 pooled eight randomised trials with 6,615 participants and found text-message reminders increased attendance compared with no reminder, and worked about as well as phone-call reminders. The evidence was rated low to moderate quality and the trials used text messages, not WhatsApp, so measure your own no-show rate.

When should a clinic send a WhatsApp appointment reminder?

Send two messages at most: a confirmation when the appointment is booked, with date, time, doctor and address, and a reminder the evening before with Confirm and Reschedule buttons. That timing is a suggestion for most outpatient clinics, not a Meta rule.

How much does a WhatsApp appointment reminder cost in India?

An appointment reminder is a utility template at ₹0.115 per delivered message in India, before GST, on the rate card effective 1 October 2026. A clinic with 40 appointments a day over 26 working days, sending a confirmation and a reminder for each, sends 2,080 templates a month, about ₹239.

What should a clinic leave out of a WhatsApp reminder?

Leave out the diagnosis or reason for the visit, test names and results, treatment or procedure names, and the department when its name gives away the condition. Messages show on lock screens and shared family phones. Leave out offers and health tips too: Meta can then price the whole template as marketing.
